Training -

1.  I understand that all behaviors on the Training Plan and/or Class Syllabus will be addressed in class. Any additional behaviors will be agreed upon by both parties.

2. I understand and agree that I am being trained to train my dog(s) and that without full cooperation with the trainer, the dog(s) will not be trained.

3. I understand that the majority of training occurs outside of class and that I will need to work with my dog(s) a minimum of 15 minutes a day in order to see results.

4. I understand that the pace of the private lesson is determined by many factors including but not limited to: the dog’s temperament, the severity of problem behaviors performed by the dog, the amount of time spent practicing behaviors, the consistency and accuracy of the owner(s) performing the behaviors.

5. I understand that the pace of the group lesson is predetermined, and it is up to me to prepare between classes in order to maintain this pace. I understand that if I am struggling with a behavior, I should alert the trainer and schedule a private lesson or short session before or after a group class.

6.  I understand that off-leash behaviors are initially taught in a familiar environment with no distractions.  In order for the dog to attain off-leash control with distractions, the dog must first attain on-leash control with distractions.

7. I understand that while training increases the chances of my dog(s) preforming a desired behavior when asked, other factors such as distractions, environment, and other stimuli may interfere with my dog's ability to successfully preform the behavior

8. I understand that it is my responsibility to ensure my dog(s) safety and set my dog(s) up for sucess in all environments, regardless of training scenario.

9. I understand that all rules and directions given by the trainer are for the safety of myself, my dog(s), and others. I understand that any injury or damage caused by my failure to follow these rules and directions is my responsibility and not the responsibility of the trainer.

10. I understand that the safety of myself, my dog(s), and others takes priority in all training scenarios and my trainer will not use any tools or techniques meant to cause intentional and/or foreseeable harm, pain, or discomfort to any party.

11. I have made my trainer aware of any known behavioral or medical conditions that may affect our training prior to beginning training sessions.

12. I understand that my trainer reserves the right to terminate the training contract in the event of unsafe training conditions.

13. I understand that deposits are non-refundable and may only be differed to other services upon approval.

14. I understand that the trainer must be given a 48-hour notice for cancellations, or I may be charged a $20 cancelation fee.

15. I understand that I may be charged a predetermined travel fee if the trainer’s travel distance exceeds 15 miles one way.

Nutrition - 

1. I understand that nutrition advice received does not replace regular veterinary care.

2. I understand that Top Dog Behavior and Training LLC's pet nutritionist is not a veterinarian and cannot diagnose or treat medical conditions.

3. I understand that changes in diet should be discussed with a veterinarian.

4. I have made Top Dog Behavior and Training LLC's pet nutritionist aware of any known behavioral or medical conditions that may affect nutritional recommendations.

5. I understand that it is my responsibility to monitor my pet(s) health regardless of diet and promptly alert my veterinarian of any changes in health condition.

6. I understand that my pet's safety and health take priority and I may be differed to a veterinarian or veterinary nutritionist if my pet's needs require further intervention.

7. I understand that the advice Top Dog Behavior and Training LLC's pet nutritionist is intended to provide guidance, but decisions regarding my pet's diet are determined by myself and appropriate parties.

As the legal owner/agent of the above-mentioned pet(s), having carefully read and fully understanding this agreement, do hereby waive and release Top Dog Behavior and Training, LLC from any and all liability of any nature.  This includes any injury, death, sickness or damage my pet may suffer during or after any service.  I also agree to indemnify and hold harmless Top Dog Behavior and Training, LLC from any and all claims due to damage the pet may cause to any family members or any third parties during or after services rendered.
